Kotaro is the kindest character inSekiro: Shadows Die Twicebut his sidequest revolves around him trying to escape a dark past. Wolf can send him to a few different fates, none of which are particularly nice.
The story ofSekiro: Shadows Die Twicerevolves around immortality and the depravity people are willing to go to to go to it. Arguably the darkest example of this is the Senpou Monks, who kidnapped and experimented on children. Kotaro earnestly cared for all the children who were experimented on, but was unwittingly party to their evil deeds.

While some might not feel much sympathy for Kotaro, he is also a key part in gettingone ofSekiro’s special endings. Whether it’s helping him out or delivering him some comeuppance, Wolf can ultimately get a lot out of his side quest.
Should Kotaro Get the White Pinwheel or Red and White Pinwheel?
Players should give Kotaro the White Pinwheel if they want the Return ending or want a “happy” ending for Kotaro, but can give him the Red and White Pinwheel to advance other sidequests.
Players have the choice of giving Kotaro either the White Pinwheel or the Red and White Pinwheel during his quest. Both pinwheels are found in remote locations in the Mt. Kongo area of Senpou Temple and can immediately be brought to Kotaro to advance his questline.

What Rewards Does the White Pinwheel Give From Kotaro?
If players give the White Pinwheel to Kotaro, they can get the Taro Persimmon item which can be used to advance the Return ending’s quest line.
When Wolf gives Kotaro the White Pinwheel to Kotaro,Kotaro asks to be spirited away. This requires to beat one of theLong-Arm Centipede Giraffe mini-bossesto gain the Divine Abduction Shinobi Prosthetic Tool.

Using the ability on Kotaro sends him to the boss arena ofthe Folding Screen Monkeys. There, he greets Wolf, gives him a Taro Persimmon, and is seemingly reunited with the spirits of the children killed by the Senpou Monks.
After a certain point, the Divine Child of Rejuvenation becomes sick and requests a Persimmon to heal her. While there are a number of Persimmon items in the game, there is a limited number and Wolf can use them in battle to rapidly regain posture.
The Taro Persimmon can be given to her instead of a regular Persimmon. If players cannot give the Divine Child of Rejuvenation a Persimmon, the questline cannot be advanced and the Return ending moves out of reach.
Should Kotaro Go to Anayama or Doujun?
If players give Kotaro the Red and White Pinwheel, they can instead send him to either Anayama or Doujun. Both are seeking the aid of a tough man, but for very different reasons. Both of these options ultimately result in Kotaro’s death.
Sending Kotaro to Anayama offers greater rewards as it expands his inventory and eventually unlocks the Promissory Note item that gives a discount from all merchants. If he goes to Doujun, players receive abundle of upgrade materialsand advances Doujun’s questline, which offers a unique Red Lump item.
