As the iconic antagonist ofDiablo 4, Lilith plays a huge part in the game’s story and is the driving force behind the chaos and turmoil in Sanctuary. Although she has been referenced in past entries of the series, Lilith’s first on-screen appearance inDiablo 4marks a major shift in the series away from the titular Lord of Terror as the primary antagonist of the game. Lilith’s role in the franchise is no less important, but her actions throughout this latest entry’s story leave the lingering question of her motivation for leading the people of Sanctuary to sin.
Known as the Daughter of Hatred, Lilith is the daughter of Mephisto, Lord of Hatred, who is one of the three Prime Evils alongside Diablo and Baal. Although she hails from the Burning Hells by birth, Lilith is not always aligned with their interests, seeking to escape the Eternal Conflict with the High Heavens. To this end, she aids in thecreation of Sanctuaryalong with the angel Inarius as a refuge away from the war, laying the foundation for the events ofDiablo 4that would see Lilith take a twisted turn away from this initially noble endeavor.

Understanding Lilith’s Motivations in Diablo 4
To understand the full extent ofLilith’s sinful quest inDiablo 4, one must start by understanding her motivation for the founding of Sanctuary. She met and fell in love with the angel Inarius in the distant past of the Eternal Conflict after discovering they shared the same wish to abandon the endless war. In an unprecedented move, the two formed a union and used the Worldstone to create a new world, Sanctuary, free from the war where like-minded angels and demons could seek refuge from the forces of the High Heavens and the Burning Hells.
The union ofLilith and Inariuswould eventually lead the two to bear offspring, as did their fellow angels and demons, establishing a new race of beings known as the nephalem. As the power of the nephalem grew, some among the angels and demons of Sanctuary wished to wipe out their creations fearing they would be overpowered or draw the attention of the High Heavens and the Burning Hells. This thought enraged Lilith, and she was driven to madness seeking to kill any who sought harm to the nephalem, leading Inarius to banish her from Sanctuary and use the Worldstone to weaken the nephalem.
As their powers weakened, the nephalem would eventually give birth to the human race as a much weaker, mortal version of their ancestors. After her banishment from Sanctuary, Lilith no longer wished refuge from the Eternal Conflict, but to end it entirely, destroying both the High Heavens and the Burning Hells in her quest. to accomplish this task, she needed to raise an army of loyal followers, and the ritual performed byElias to bring Lilith back to Sanctuarygave her the perfect opportunity to enact her plan and end the Eternal Conflict once and for all.
Lilith, now back in Sanctuary, intends to corrupt its human inhabitants to her side by unlocking the darkness within their hearts through the power of sin and using them as tools against the forces of Heaven and Hell. The majority of Sanctuary worships theCathedral of Lightand its figurehead Inarius, so Lilith must tempt his followers by offering them salvation from the coming conflict by submitting to her and worshiping her as their goddess. As Lilith’s influence spreads, her power grows and enables her demonic followers to overtake Sanctuary and further will humanity under the control of the Daughter of Hatred as she prepares to strike against Heaven and Hell.
